Service Description:
|
- Offers health services and a variety of health programs
- Provides access to other health care professionals within and outside of the Family Health Team
- Coordinates documentation if patient receives care from a member of the team
Administers the following programs and services:- Chronic Obstructive Pulmonary Disease (COPD) or Asthma Program
- Diabetes Self-Management Program
- 18-Month Well-Baby Visits
- Hypertension Program
- INR Management Program
- Memory Clinic
Also provides access to services through the following community partners:- Ontario Health atHome - North West Area
- Northwestern Health Unit
- Community Counselling and Addiction Services
- CAMH Centre for Addiction and Mental Health
