Service Description:

The Sunset Country Family Health Team includes: Physicians, Nurse Practitioners, Registered Nurses, Registered Practical Nurses, an Occupational Therapist, Dietitians, Pharmacist, Chiropodist, Social Worker, Consulting Psychiatrist and Administration. Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy.
Offers Occupational Therapy (OT) services by a registered Occupational Therapist
Provides assessments and recommendations for:- home safety
- daily living skills
- mobility aids and adaptive equipment
- cognition
- falls prevention
- sensory
- chronic disease and pain management
- return to work
- ergonomic
- stroke rehabilitation
