Service Description:

The Sunset Country Family Health Team includes: Physicians, Nurse Practitioners, Registered Nurses, Registered Practical Nurses, an Occupational Therapist, Dietitians, Pharmacist, Chiropodist, Social Worker, Consulting Psychiatrist and Administration. Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy.
Assists patients living with asthma or Chronic Obstructive Pulmonary Disease (COPD) to better manage their disease processConducts spirometry assessments to determine lung functionProvides education and support to help set goals for individual's careOffers smoking cessation counselling if neededArranges follow-up visits as neededProvides referrals to other programs and services
