Service Description: The Sunset Country Family Health Team includes: Physicians, Nurse Practitioners, Registered Nurses, Registered Practical Nurses, an Occupational Therapist, Dietitians, Pharmacist, Chiropodist, Social Worker, Consulting Psychiatrist and Administration. Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy.

Supports for individuals living with pre-diabetes or diabetes
Offers a diabetes management clinic to assist individuals in understanding and managing their illness
Services offered:
  • Diabetic screening
  • Pre-diabetes clinics
  • Prevention and awareness programs
  • Education on diabetes (insulin, glucose and nutrition)
  • Promotes understanding of diabetes management and self care (type of diabetes and relationship between nutrition, exercise, medications and stress)
Programs and services also include:
  • School programs and visits
  • Client support groups
  • Foot care workshops
  • Presentations to the public and health care providers
  • Grocery tours
Program services are provided through:
  • Individual sessions
  • Group sessions
  • Health promotion activities

Fees: None
Application: Book appointment online or call office; no referral or primary care provider required
Eligibility / Target Population: Individuals 18 years of age and older living with:

  • Pre-diabetes

  • Type 1/Type 2 Diabetes

  • Gestational Diabetes

  • Type 1/Type 2 Pregnancy

  • Impaired Glucose Tolerance

  • Impaired Fasting Glucose

  • High Risk
