Service Description:
|
Family Health Team includes: Family Physician, Registered Nurses, Registered Practical Nurse, Social Worker, Registered Dietitian, Addiction Program Care Coordinator, Administration and other professionals. Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy.
Supports for people living with pre-diabetes or diabetes- Assists individuals to cope with diabetes and prevent complications
- Offers education and self-management support to fit an individual's lifestyle
- Facilitates client support groups
- Provides access to a Registered Nurse and Registered Dietitian
- Coordinates foot care workshops

Individuals 18 years of age and older with type 1 or type 2 diabetes, or have a high risk of developing diabetes
